How to fill out an LOA

In this article you we will explain in detail what information you will need to fill in on an LOA.

What is an LOA?

An LOA (Letter of Agency/Authorization) is a document required to give one carrier permission to port numbers from the current carrier on behalf of the end-user.
The LOA must be filled out completely and must have a legible signature from the authorized person on the account, otherwise it may be rejected.

If you are unsure of the information required on the LOA, please reach out to your current carrier for a CSR( Customer Service Record). A CSR holds all of your account information and will help you with filling out the LOA accurately. 

Account or Company Name 

This is the name of the end-user account or company name on record with the current carrier

4 Address Fields

Street w/ Number, City, State/Province, Zip/Postal Code

These fields should be the service address on file for the number(s), not the billing address (unless they are the same). Please verify this with information the losing carrier or request a CSR from the losing carrier in order to validate the address.

Carrier Name

This is the current carrier’s name. 

Billing Telephone Number (BTN)

This should be the BTN on record with the losing carrier. This is 1 individual telephone number. This is usually the number on record on the account with the losing carrier. 

Numbers to be ported

The number(s) being ported need to be put in NPA-NXX-XXXX format and separated by commas. For a large amount of TNs, please ask attach an excel spreadsheet with the numbers in each cell in each row within the same column

Authorized Signature

This is the signature of the authorized person on the account. This must be clear and legible.

The authorized person’s name in capital/block letters

Date

The date that the LOA was agreed to and signed on
